You should contact your local telephone company to get an application.
- BellSouth customers should call (888) 757-6500
- Nextel Partners customers should call (888) 566-6411 or email lifeline@nextelparners.com
- Alltel customers can visit their nearest Alltel retail store

Proof that you get one of the following benefits:
- Temporary Assistance to Needy Families (TANF)
- Food Stamps
- Medicaid or SSI
- Senior citizen local gas discount plan
- Federal Public Housing Authority (FPHA)
- Low-Income Home Energy Assistance Program (LIHEAP)
You may also be asked to provide:
- Proof of Social Security number
- Proof of age
