How do I apply for Low-Income Medicaid?

Call or go to your county Department of Family and Children Services (DFCS) office to apply. DFCS can take your application or help you find other places to apply such as schools, hospitals or community centers.

After you complete an application, you’ll have to interview with a caseworker at the Medicaid office.

You’ll find out if you’ll get Medicaid within 45 days after you apply.

What do I Need to Bring?
To apply for this benefit, you will need:
  • Proof of age for yourself and your children
  • Proof of Social Security Numbers for yourself and your children
  • Information on your or your children’s current or past health insurance such as an insurance benefit card or insurance policy number
  • Proof of your household’s income
  • Proof of citizenship or immigration status for yourself and your children
  • Proof of life insurance policies
  • Three most recent bank statements or bank books
  • Proof of property you own (such as land, stocks or bonds), or property you plan to buy
